Procedures performed routinely as first assist, together with the perioperative and critical care that surrounds them.
Adult cardiac — first assist
Coronary artery bypass grafting, including multi-vessel and arterial revascularization. Aortic, mitral, and tricuspid valve replacement and repair. Aortic root, ascending, and hemiarch replacement, including repair for acute dissection. Reoperative cardiac surgery. Minimally invasive valve approaches. Atrial ablation procedures. Septal defect closure.
Thoracic — first assist
Video-assisted and robotic thoracic surgery, including anatomic lung resection — lobectomy, segmentectomy, wedge, and pneumonectomy. Mediastinal and regional lymphadenectomy. Thymectomy and mediastinal mass resection. Decortication and pleurodesis. Foregut surgery, including paraesophageal hernia repair, Heller myotomy, and esophagectomy assist.
Conduit harvest
Endoscopic saphenous vein harvest. Endoscopic and open radial artery harvest. Open saphenous harvest.
Cannulation and mechanical support
Central and axillary cannulation. Femoral artery and vein cutdown for minimally invasive and peripheral approaches. Intra-aortic balloon pump, Impella, durable LVAD, and ECMO experience.
Critical care and bedside procedures
Primary management of postoperative cardiac and thoracic ICU patients. Central and arterial line placement, chest tube insertion and removal, Swan-Ganz manipulation, thoracentesis, and pericardial drainage.
Course Chair, First Assist training.
Since 2018, Lance has served as West Coast First Assist Course Chair for Intuitive Surgical's robotic thoracic surgery program — responsible for the design and delivery of first assist training, including didactic instruction and hands-on cadaveric and porcine laboratory teaching.
He also co-hosts surgeon training courses at the TR200, TR300, and TR400 levels, at course sites in Sunnyvale and San Diego, California; Seattle, Washington; and Phoenix, Arizona.
